Your Rights as a Patient

  • To be treated with dignity, courtesy, and respect.
  • To receive information about your care in a manner you can understand.
  • To participate in decisions about your treatment and ask questions about recommended options.
  • To receive information about material risks, benefits, and alternatives when appropriate.
  • To refuse treatment, subject to applicable law, and to be informed of reasonably foreseeable consequences.
  • To privacy and confidentiality consistent with applicable law.
  • To request access to your records as permitted by law.
  • To voice concerns or complaints without retaliation.

Your Responsibilities as a Patient

  • Provide accurate and complete information relevant to your care.
  • Update the practice about important changes in symptoms, medications, health conditions, contact information, insurance, or location.
  • Ask questions when instructions or treatment recommendations are unclear.
  • Participate in the agreed treatment plan and discuss barriers that may affect your ability to follow it.
  • Keep scheduled appointments or provide reasonable notice when cancellation or rescheduling is necessary.
  • Use respectful communication with staff and providers.

Medication Safety

Patients are responsible for providing an accurate medication list, including prescription medications, over-the-counter products, and relevant supplements. Do not change, stop, share, or increase psychiatric medications without discussing the change with an appropriate prescriber unless emergency instructions direct otherwise.

Telehealth Responsibilities

For telehealth visits, patients should attend from a private location when possible, maintain a reliable internet connection, provide their physical location at the time of the appointment, and identify an emergency contact when requested.

Safety and Emergencies

Brightwave Psychiatry PLLC is not an emergency service. If you are experiencing a medical or psychiatric emergency or believe someone is in immediate danger, call 911 or go to the nearest emergency department. In the United States, call or text 988 for the Suicide & Crisis Lifeline.
