  • Obtain a paper or electronic copy of this notice.

Our Uses and Disclosures

We may use and disclose protected health information for treatment, payment, and healthcare operations as permitted by law. Examples include coordinating your care, billing or payment activities, quality improvement, practice administration, and other lawful healthcare operations.

Other Permitted or Required Uses

Health information may also be used or disclosed when permitted or required by law, such as for public health activities, health oversight, certain judicial or administrative proceedings, law enforcement purposes, preventing serious threats to health or safety, workers’ compensation, or other legally authorized purposes.

Uses Requiring Authorization

Uses and disclosures not otherwise permitted by law generally require your written authorization. You may revoke an authorization in writing, subject to actions already taken in reliance on it.

Our Responsibilities

  • We are required to maintain the privacy and security of protected health information as required by applicable law.
  • We will notify affected individuals when required following certain breaches of unsecured protected health information.

Complaints and Questions

You may raise privacy questions or complaints directly with the practice. You may also have the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. Brightwave Psychiatry PLLC will not retaliate against you for filing a complaint.

Effective Date and Updates

The practice may change the terms of this notice as permitted by law. A revised notice may apply to health information already maintained as well as information received in the future.
